Home Care Coordinator

A local home care agency, HomeAides Inc., is looking for a compassionate and organized Home Care Coordinator to oversee and support the daily delivery of home health and personal care services. This person will make sure clients receive quality, compliant, and personalized care while keeping communication open between families, caregivers, and healthcare providers. This role is perfect for someone who enjoys working with people, thrives in a busy environment, and takes pride in helping clients stay comfortable and cared for in their own homes.

Responsibilities
  • Client Intake & Assessment

    • Conduct initial client assessments to determine care needs and eligibility for services.

    • Develop and implement individualized care plans in collaboration with nurses, physicians, and family members.

    • Reassess client needs regularly and update care plans as needed.

  • Care Coordination

    • Schedule and assign caregivers based on client needs, staff availability, and geographic location.

    • Monitor service delivery to ensure compliance with care plans, quality standards, and regulatory requirements.

    • Communicate regularly with clients and families to ensure satisfaction and address any concerns.

    • Coordinate with external providers (e.g., hospitals, clinics, social services) to facilitate transitions of care.

  • Staff Supervision & Support

    • Recruit, train, and supervise home care aides and nurses.

    • Provide ongoing coaching, performance evaluations, and support to field staff.

    • Ensure all caregivers maintain required certifications and training.

  • Administrative & Compliance

    • Maintain accurate and timely documentation in client records and agency systems.

    • Ensure compliance with HIPAA, state licensing, and agency policies.

    • Prepare reports on service utilization, client outcomes, and staffing.

    • Participate in quality assurance and improvement initiatives.

Qualifications and Skills
  • High School Diploma or equivalent

  • Bachelor’s degree in Nursing, Social Work, Healthcare Administration, or related field a plus but not required

  • Strong attention to detail and organizational skills

  • Interpersonal & communication skills

  • Proficient in Microsoft Applications (Word, Excel)

  • At least 1 year management experience

  • Caregiving background a plus

  • Valid Driver’s License

  • Proof of Auto Insurance

  • Must be able to pass all background checks and clearances required by the state

Benefits/Compensation
  • Full Time

  • First shift (8am – 5pm)  

  • $18+/hr based on experience

  • Direct Hire Role

  • Weekly pay while employed through Career Concepts
